Also called a durable power of attorney for health care, health care proxy or medical power of attorney, this is a legal document in which you name a person as your `health care agent' to make medical decisions for you if, at some future time, you are unable to make your own. Unless you formally appoint someone to decide for you, many health care providers and institutions will make critical decisions for you that might not be based on your wishes. In some situations, a court may have to appoint a guardian unless you have already done so.
