Case Management (Adolescent Mental Health)

A service that helps people arrange for appropriate services and supports. A case manager coordinates mental health, social work, educational, health, vocational, transportation, advocacy, respite care, and recreational services, as needed. The case manager makes sure that the changing needs of the child and family are met. (This definition does not apply to managed care.)

Case Management (Employee Benefits)

The process through which covered persons with specific health needs are identified and counseled to achieve the most appropriate levels of service utilization and optimum treatment outcomes.

Case Management (Family Health)

A program in which a case manager (physician, nurse or other healthcare professional) monitors a patient who requires long-term medical care due to a catastrophic illness or injury and works with the patient’s attending physician to help improve the continuity, quality and cost efficiency of care.

Case Management (Health Care)

A system for assessing, planning treatment for, referring, and following up on patients in order to ensure the provision of comprehensive and continuous service and the coordination of payment and reimbursement for care.

Case Management (Health Insurance)

A process of identifying plan members with special healthcare needs, developing a health-care strategy that meets those needs, and coordinating and monitoring care.

Case Management (Insurance)

Coordination of patient care to ensure appropriate care and reductions in costs of providing services. Physician case managers coordinate such elements as referrals to consultants, specialists, hospitals, ancillary providers and services. This is intended to eliminate misutilization of facilities and resources, fragmented services and to provide continuity of services and intensity of services appropriate to the patient's needs over time.

Case Management (Long Term Care Insurance)

A professional service which arranges and coordinates health and/or social services through assessment, service plan development and modification, monitoring, and quality assurance.

Case Management (Medical Education)

Coordination of services to help meet a patient's health care needs, usually when the patient requires multiple services from multiple providers. This term is also used to refer to coordination of care during and after a hospital stay.

Case Management (Mental Health)

A service that helps people arrange for appropriate services and supports. A case manager coordinates mental health, social work, educational, health, vocational, transportation, advocacy, respite care, and recreational services, as needed. Managed care definition: A system requiring that a single individual in the provider organization is responsible for arranging and approving all devices needed under the contract embraced by employers, mental health authorities, and insurance companies to ensure that individuals receive appropriate, reasonable health care services.

Case Management (Natural Disaster Response)

A systems approach to provision of equitable and fast service to applicants for disaster assistance. Organized around the needs of the applicant, the system consists of a single point of coordination, a team of on-site specialists, and a centralized, automated filing system.
