Ergonomic Evaluation

Ergonomic Evaluation (Workers Compensation)

State-required employer's report to be filed whenever the employer has knowledge that a worker has either sustained, or is alleging to have sustained an industrial injury or illness. The report is mandatory for admittedly compensable injuries, as well as for cases where the employer questions or denies liability for compensation. Self insured employers are required to file a copy of their reports with the State within a MAXIMUM period of (5) days of first knowledge of a claim.
