Written statement appointing a health care agent who will make all medical decisions on one’s behalf should one become incapacitated.


Health Care Proxy (Female Health)

Also called a durable power of attorney for health care, medical power of attorney or appointment of health care agent, a legal document in which you name a person to make medical decisions for you any time you are unable to make your own medical decisions. Unless you formally appoint someone to decide for you, many health care professionals and institutions will make critical decisions for you that might not be based on your wishes. In some situations, a court may have to appoint a guardian unless you have already done so.


Health Care Proxy (Health Care)

A health care proxy is recognized in some states as an alternate method for naming a person to act on one's behalf in health care decision making. In a few states, a health care proxy may be included as part of a living will.
