A form of disclosure document containing required information to be supplied by the franchisor to the franchisee. Initially promulgated by the Midwestern Securities Commissioners to provide a uniform method of disclosure for the benefit of franchisor and franchisee, its use is permitted in non-regulated states by FTC Rule 436.
